• This form is required prior to booking. Please complete, sign, and return to me before your appointment.

Custody Affidavit 

• This form is for anyone traveling out of the country with a minor child. Complete either the Sole Custody Form or the Other Parental Consent Form, have it signed and notarized, and keep it with your proof of citizenship documentation.
